Notice of Right to Opt-Out for California Residents and Webform for California Consumer Privacy Act (“CCPA”) Data Subject Requests

For California Residents Only - Beginning January 1, 2020, California residents have the right to opt out of the sale of personal information. Personal information includes data such as your name, postal or email address, telephone number, and other personally identifying information. The law does not apply to non-personally identifying information such as data that has been aggregated and de-identified. It also does not apply to our site’s service provider partners who need the information to perform certain business functions. For example, if you apply for an insurance plan from our website, the insurance underwriter needs the personal information in order to provide and service your insurance policy. You may request deletion of your personal information, however, we may not be able to fulfill your request if you have applied for insurance, created an account, communicated a customer support request via email/messaging/telephone with us, or purchased other products from us. Our obligations under other federal rules governing insurance, as well as our data retention policies, may prevent us from removing your information. However, you may clarify you wish to opt out of future marketing communications, which we will honor. If you have an active insurance policy with us, we may be required to send you communication to provide customer support.

If you wish to opt-out of the sale of your personally identifiable information, you may do so by filling out the form below.

For your convenience, we provide the following options that you can check/select if they adequately reflect the nature of your CCPA related request. If you select one or several of these options and do not provide further information about your CCPA request, we will assume that your request relates solely to the option or options you selected, and address it accordingly (in each case subject to any available legal exceptions):

Subject to available exceptions under Section 1798.105 (d) of the Cal. Civ. Code, please delete all personal information that you collected from me.

Please disclose the categories of personal information collected about me, the categories of sources from which the personal information is collected, the business or commercial purpose for collecting personal information, the categories of third parties with whom you share personal information, and the specific pieces of personal information collected about me.

For options 2, 3,and 4 we will ask that you provide certain information to verify your identity, such as a code sent to an e-mail address we may have on file for you. We may ask for such information also for option 1. The information that we ask you to provide to verify your identity will depend on your prior interactions with us and the sensitivity of the personal information at issue. For Option 2, once we have verified your identity, we will ask you to confirm that you wish to have personal information deleted. Once confirmed, we will respond to your request in accordance with the CCPA. You can learn more about the right to opt-out of the sale of personal information and the use of an authorized agent to submit requests on a consumer’s behalf in our CCPA Privacy Policy. If we deny your request, we will explain why.
